이건 수혜자(Beneficiary)인 우리가 작성할 필요는 없고 청원자(Petitioner)인 미시민권자가 작성하는 서류인데 이걸 서류 접수할 때 신청하면 신청비는 $0. 추후에 신청하고 싶다고 해도 신청 할 수 없음. 나중에 케이스가 진행될 때 문자(당연히 미국내 번호로 전달받게 된다)나 이메일로 진행상황을 전달받고 싶다는 신청서라고 한다. 신청서 폼은 다음의 웹사이트에서 다운로드 받을 수 있다. 언제나 그렇듯 폼은 언제나 웹사이트에서 최신 업데이트된 폼이 맞는지, 유효기간에 맞는 폼이 맞는지 더블체크!
